Community Hub - List of Activities
In your Community Builders service specification you will see that Community Hub includes activities you are funded for under column B. The list below shows all activities that fall under Community Hub.
LCSA has developed tools that will give you the relevant Minimum Data Set Tool for each activity. Click here to go to the Community Hub Minimum Data Set Tools.
- People connected with information - Provision of quick and easy access to information about local services and resources to individuals either by phone, e-mail, brochures, fact-to-face etc... You will have to collect and report data for the number of people connected with info (detailed by category and type of contact), SNAPSHOP DATA ONLY
- Clients Assisted with Referral - Linking a person with another service by contacting a service on behalf of a client, support the client to contact a service, discuss options with a clients or support in writing a letter. You will have to collect and report data for number of referrals made during snapshot reporting period.
- Using community infrastructure - Capturing the usage of the centre and its facilities and other local community facilities e.g. hall bookings, using the centres photocopier, computers, fax etc... You will have to collect and report data for the number of occasional bookings and number of occasional uses of the centre's resources. (SNAPSHOP DATA ONLY)
- Services coordinated through the Hub - Ongoing services, programs and activities coordinated through the Hub e.g. yoga, tax help etc...Some services may list here also the services that operated out of the centre yet receive funding from other sources or are outreach services of another agency. You will have to collect and report data for the number of ongoing services, programs and activities e.g. yoga, taxhelp etc... (WHOLE OF REPORTING PERIOD DATA)
- Community consultation - your centre organises or facilitates a community consulation, a community forum or a public meeting. You will have to collect and report data for the number of activities and the number of participants.
- Community Plans - your centre initiates or organises developed as a result of community consultations e.g. a service delivery plan. You will have to collect and report data for the number of activities and the number of participants.
- Events - your centre organises or provides a leadership role in organising an event e.g. open day, expo of local services. You will have to collect and report data for the number of events held and the number of participants.
- Networks established or facilitated - your centre leads or facilitates a network or interagency meeting for services around a specific issue or for peer support. You will have to collect and report data for the number of organisations participating as well as the frequency of meetings.
- Partnership project - your centre establishes or leads a collaboration around an issue identified to effect the community on a local or regional level. You will have to collect and report data for the number of project and the number of participants.
- Integrated plans - your centre establishes or leads regional/network planning activities e.g. community saftey plan, Neighbourhood Centres intergrated plan. You will have to collect and report data for the number of plans and the number of participating organisations.
